Voluntary benefit that covers eye exams, lenses or contacts every 12 months and frames every 24 months

Short-term Disability (STD) Salary Continuation
Available to eligible associates after 90 days of active employment
Coverage provided at no cost to the associate
Temporarily disabled associates may receive up to 100% of base pay, for a maximum benefit duration of 12 weeks
Long-term Disability (LTD) Insurance
Automatic enrollment for benefits eligible associates after 90 days of employment
Coverage provided at no cost to the associate through a salary gross up
Benefit is 60% of base salary
Cancer and Specified Disease Insurance
Benefits are paid directly to associate
Annual wellness benefit offered
Coverage available to benefits eligible associates and their dependents
Long-term Care Insurance
Guaranteed issue coverage for benefits eligible associates as at initial eligibility
